We regularly support people coming home from King's College Hospital on Denmark Hill, run by King's College Hospital NHS Foundation Trust, which serves residents right across Southwark, as well as those discharged from Guy's Hospital near London Bridge, run by Guy's and St Thomas' NHS Foundation Trust. Getting the first few weeks at home right matters, so we help with medication routines, mobility and rehabilitation support, and we keep in close contact with families and district nurses to spot any problems early.

If you think a relative may qualify for help with funding, you can ask the council for a free care needs assessment, followed by a financial assessment.
