The first weeks back at home matter, so we help with medication routines, mobility and rehabilitation support and stay in close contact with families and the Trust's community teams.
In Northern Ireland health and social care are integrated, so adult social care in Armagh is arranged by the Southern HSC Trust rather than by a council.
